How to send a referral for Healthy Families

1) Go into All forms (where you fill out all the paperwork for your client)

2) Click on “Healthy Families” at the top of the page

3)Check “Yes”: On the referral form, ensure you check the “Yes” option.

4)Complete All Fields: Fill in all the required fields on the page.
5)Click Save & Continue: Scroll to the bottom of the page and click the “Save & Continue” button. When you click this it will automatically submit the form to Healthy Families.
